all i can say is that you need to *tell* them as much as you can about what it is like. the things you mention here, that medical exams don't have check-boxes for.

like "it takes me two days to recover from a simple activity like buying food for mysef."

i made sure to make things like that very clear and very real to the people weighing my case. i did everything i could to not make it easy for them to shrug it off.
